An Initial Guide For ORTHODONTICS

Orthodontics is a special branch of Dentistry that treats patients with improper positioning of teeth. The word 'orthodontics' or 'orthodontia', as it was previously called, comes from the Greek words 'ortho', meaning 'straight or perfect' and 'dontos', meaning 'teeth'.  Orthodontics also deals with unusual shape and development of jaw or improper facial growth (dentofacial orthopedics). A person who specialize in orthodontics is called an orthodontist. It is a form of cosmetic dentistry which is used to improve the appearance of a person.



Orthodontics treatment is valuable for treating the following:

1) Treating an improper bite or Malocclusion.

2) Straightening crooked teeth

3) To improve oral functions such as speech or eating.

4) Closing wide gaps between the teeth.



Malocclusion or improper bite is of many types. It may be overbite, where the upper front set of teeth protrudes out over the lower set which may injure the person in sports, falling down, etc. Then, there is underbite, where the lower front teeth are too far forward or the upper teeth are too far back and crossbite and openbite, when the upper front teeth does not come down over the lower teeth while biting normally and there is space between the biting surfaces of the front or side teeth when the back teeth bite together, respectively. There may be misplaced midline where the center of your upper front teeth does not line up with the center of your lower front teeth. Cawding is also one form of malocclusion where the patient's jaw is narrow and there may not be enough space for all the teeth.



Orthodontics treatment treats all the above mentioned dental problems.  Different types of appliances, fixed or removable, are used in orthodontics treatment. The most commonly used fixed appliance is braces which consists of wires or brackets and bands. Bands are fixed firmly around the teeth and act as anchors for the appliance, while the brackets are bonded to the front of the tooth. Arch wires pass through the brackets and are attached to the bands. This arch wire when tightened, puts tension on the teeth, thus, gradually moving them to proper positions. The braces are adjusted once a month and the treatment time varies from seven month to several years. 



Other than braces, commonly used fixed appliances are fixed space maintainers and special fixed appliances.  Fixed space maintainers are used when a child looses a milk tooth to stop the two teeth from the two sides to come inside the gap until the adult teeth comes out. Special fixed appliances are useful for patients to control tongue thrusting or thumb sucking, which they may find uncomfortable while eating.



Other removable appliances used for an orthodontist treatment includes aligners, removable space maintainers, jaw repositioning appliances, lip and cheek bumpers, palatal expander, removable retainers and head gears.

An orthodontist will do an initial diagnosis which includes a full medical and dental health history, a clinical examination and X-rays of teeth and jaw. Sadly, you need to have a root canal. Great. But, what exactly is that going to entail? Do you need a driver? Is it going to be painful? Why did this happen in the first place? Well, a root canal will require one or more office visits, depending on the severity of the case. It will either be performed by a dentist or endodontist. We all know what a dentist is, but an endodontist is a dentist who specializes in the diseases and injuries of the human dental pulp or the nerve of the tooth. The choice of which type of dentist to use depends to some degree on the difficulty of the root canal procedure needed in your particular tooth and the general dentist's comfort level in working on your tooth. After a brief consultation, your dentist will be better able to tell who might be best suited to perform the work in your particular case.

At your appointment, you will take an X-ray to see the shape of the root canals and determine if there are any signs of infection in a surrounding bone. Once everything is ready for surgery, your dentist or endodontist will then use local anesthesia to numb the area near the tooth. Anesthesia may not be necessary, since the nerve is dead, but most dentists still anesthetize the area to make the patient more relaxed and at ease. The anesthesia used will determine whether or not you will need to bring someone with you to drive you home.

Next, to keep the area dry and free of saliva during treatment, your dentist will place a rubber dam (a sheet of rubber) around the tooth. Once your mouth is prepared, an access hole will then be drilled into the tooth. The pulp along with bacteria, all of the decayed nerve tissue and other debris is then removed from the tooth using root canal files. A series of these files of increasing diameter are each subsequently placed into the access hole and worked the full length of the tooth to scrape and scrub the sides of the root canals. Water or sodium hypochlorite is used periodically to flush away the debris.

Once the tooth is thoroughly cleaned, it is sealed, which may take up to a week, depending on the doctor. If there is an infection, your dentist may put a medication inside the tooth to clear it up. Others may choose to seal the tooth the same day. If the root canal is not completed on the same day, a temporary filling is placed in the exterior hole in the tooth to keep out contaminants like saliva and food between appointments.

At the next appointment, to fill the interior of the tooth, a sealer paste and a rubber compound called gutta percha is placed into the tooth's root canal. To fill the exterior access hole created at the beginning of treatment, a filling is placed. Once filled, you may have a tooth restoration procedure, but that's it. You're done and no more toothache. Because a tooth that needs a root canal often is one that has a large filling or extensive decay or other weakness, a crown, crown and post, or other restoration often needs to be placed on the tooth to protect it, prevent it from breaking, and restore it to full function. Your dentist will discuss the need for any additional dental work with you. Pain will be involved, but you are already in pain. This pain will eventually go away and your tooth will be good as new.

Ever since we were old enough to have candy, we have been more than delighted by the offer of a stick of gum. Some of the best odor-fighting flavors are cinnamon, citrus and any minty flavor you can get your hands on. But beyond the fresh burst of flavor, chewing gum also perpetuates saliva production. Saliva helps fight against dry mouth and the odors created as it helps clean your teeth and stave off sugary snacks in between meals, thus further combatting bad breath.

While there is lots of sugary gum is available on the market, there have also been advances in chewing gum production that can help PREVENT cavities and promote other health benefits than the more cavity-inducing versions. The substitution of Xylitol for sugar has not only reduced cavity production, but it also attacks micro-organisms that damage teeth so our teeth can re-mineralize faster. While the detrimental effects of periodontal disease, commonly known as gum disease, have been carefully documented and highly publicized within the last few years, what people still don’t seem to realize is how bad periodontal disease is for you.

  3. It may affect your breathing. Breathing in harmful bacteria has been known to cause respiratory infections, so what if that bacteria is living in your mouth? Periodontal disease may increase your risk of developing respiratory infections, such as pneumonia.

  4. It can make blood sugar harder to control. People that have diabetes are more likely to have gum disease, than those who don’t, and inflammation may be partly to blame. Diabetics are more susceptible to infections, including gum disease. Additional research has found that gum disease can make blood sugar harder to control, and that treating can help improve diabetes symptoms.

  5. It may cause problems with conception. Aside from pregnant women being more at risk to develop periodontal disease, it seems gum disease can also cause problems with conception. Those with periodontal disease may have a tougher time getting pregnant, while also increasing the chances of miscarriage.
